We provide information and advice on your rights and choices, including free Advance Decision templates as the leading provider of Advance Decisions in the UK and produce factsheets and toolkits which aim to help people understand their rights and choices.

We provide information for carers and professionals involved in the care and support of someone who is at the end of life.
We conduct research and are involved in policy related to end of life care.

Subject Definitions

For some people later in their life the cells in the brain that are related to mental ability become damaged, causing a gradual decrease in the ability to think and remember. When this happens it is referred to as dementia. Dementia is a non-specific illness syndrome (set of signs and symptoms) in which affected areas of cognition may be memory, attention, language, and problem solving.
