Routine referrals

We accept referrals from GPs.

Click here for the Non AQP referral form  (previously known as 'direct referral form to audiology for hearing assessment')

Patients over the age of 60 with no other complications can be referred directly for a hearing assessment, and/or obtain hearing aid provision.

Patients under the age of 60 years or who breach Technicians, Therapists and Scientists in Audiology (TTSA) criteria will be referred to an ENT consultant for assessment. The patient should be referred on to audiology for rehabilitation where appropriate.

Emergency referrals

If a patient has a hearing device that is lost, damaged or broken, please telephone the audiology department on 020 7188 2211 for an appointment for repair or replacement, clearly communicating the urgency of the request.

We will also accept emergency referral requests by fax or email - please mark these as 'urgent'.

Please note: there may be a charge if the device has been carelessly damaged or lost.
